Soccer Fall 2010 Registration

On-line Registration: August 1st - September 3th
Cost: $85 per child, $75 per sibling

The registration process includes your acceptance of the following forms prior to registering your child:
 
1.  SLYAP Code of Conduct
2.  Release and Hold Harmless Agreement
3. Youth Sports Medical Release Form

Please read the agreements carefully, and fill a registration form per child.

 

 

We have setup Paypal credit card payments. After registering each child, please click on "add to cart" to add the payment item. Repeat the registration process for each additional child you want to register, and then proceed to checkout.
